Handling of Personal Information
The APPs set the framework for how we manage personal and health information collected in your patient file. Young Form Physiotherapy only collects, uses, or discloses personal information in ways that are consistent with the Privacy Act and this policy. Your patient records are treated with the highest degree of confidentiality.
Access to patient files is limited to your treating practitioner, and when required, other practitioners within the clinic. Staff may also access records to carry out essential administrative tasks. All employees are bound by strict confidentiality obligations as part of their employment and are required to handle patient information responsibly.
Collection and Use of Information
Personal information is primarily collected to provide healthcare services tailored to your needs. We will not share your information with third parties unless it is necessary for your care, legally required, or you would reasonably expect us to do so.
Information may be disclosed to entities such as Medical Defence Organisations, insurers, accreditation bodies, medical specialists, or legal representatives without patient consent when permitted by law and within reasonable expectations.
Patient Rights
You have the right to access the personal information we hold about you. While all patient records remain the property of Young Form Physiotherapy, patients may request access to their file, excluding correspondence from other medical practitioners as permitted by law.
Patient records are retained for a minimum of seven years from the last consultation. For patients under 18, records are kept until the patient reaches, or would have reached, 25 years of age.
